Özet

Bir  devletin  ekonomik  büyüme  hızı  ve  buna  bağlı  olarak  gelişen  refah  düzeyi  o  ülkenin  mevcut  sağlık  sektörü ve  ülkenin  tercih  ettiği  sağlık  sistemleri  ile  doğrudan  ilişkilidir.  Tüm  dünyada  ülkelerin  uygulamada  tercih ettikleri  sağlık  sistemleri  ve  bu  sistemlerin  özellikleri  ülkelerin  sağlık  harcamalarını  dolayısıyla  ülkenin  genel sağlık  durumunu  doğrudan  etkileyen  faktörlerdir.  Bu  yüzyılda  sağlık  teknolojilerinin  artan  oranlı  gelişimi toplumların  doğrudan  sağlık  göstergelerinde  birçok  gelişme  yaratırken  bunun  arka  planında  uygulanan  sağlık sistemlerin  payları  göz  ardı  edilemez  niteliktedir.  Bu  doğrultuda  bu  çalışmanın  amacı  sağlık  sistemlerini  teorik olarak  ortaya  koymak  ve  refah  yönelimli  sağlık  sistemini  uygulayan  ülkelerin  (Almanya,  Fransa,  Japonya, Kanada,  Brezilya,  Mısır,  Malezya,  Hindistan,  Libya)  ve  Türkiye’nin  1980-2020  yılları  arasındaki  sağlık göstergelerini  analiz  edilerek  sağlık  sistemleri  karşılaştırması  yapmaktır.  Buna  ek  olarak  ülkelerin  sağlık sistemlerinin  benzerlik  ve  farklılıklarını  tespit  ederek  Türkiye’nin  sağlık  sisteminde  geliştirilmesi  gereken boyutları  ortaya  koymak  ve  yaşanan  sorunlara  bazı  öneriler  getirilmesi  amaçlanmaktadır.    Bu  amaçla,  sağlık ekonomisi  literatüründe  sıklıkla  kullanılan  doğumda  beklenen  yaşam  süresi  ve  bebek  ölüm  hızları  sağlık göstergeleri  olarak,  sağlık  harcamaları  ve  GSMH’ye  oranları  da  ekonomik  performans  göstergeleri  olarak  ele alınmıştır.  Aynı  sağlık  sistemlerine  sahip  olan  ülkelerin  kişi  başı  sağlık  harcamaları  ve  bu  sağlık harcamalarının  GSMH’deki  payları  son  40  yıl  içinde  önemli  derecede  artmıştır.  Ancak  sağlık  sistemleri  aynı olan  ülkelerdeki  sağlık  harcamaları  farklılık  gösterirken  diğer  yandan  sağlık  harcamalarının;  sağlık göstergeleri  ve  sağlık  sistemlerinin  başarı  ve  verimlilikleri  ile  birebir  ilişki  içinde  olmadığı  sonucuna ulaşılmaktadır. 

The economic growth  rate of  a country  and  the welfare level  are directly  related  to  the current  health  sector and health systems  of  that  country.  The  health systems  preferred by  countries  and  the  characteristics  of  these systems are the factors  that  directly  affect  the health  expenditures of  the  countries and  therefore the general health  status  of  the  country. While  the  innovative  development  of  health technologies  in  this  century has created  many  developments in  the  health  indicators of  societies,  implemented  shares  of  the health  system  in the  background  of  these  cannot  be  ignored.  In  accordance  with this  purpose;  the  aim  of  this  study  is  to  reveal and  compare  the  health  systems  theoretically  by  analyzing  the  health  indicators  of  the  countries  that  apply  the welfare-oriented  health  system  (Germany,  France,  Japan,  Canada,  Brazil,  Egypt,  Malaysia,  India,  Libya)  and Turkey between 1980-2020.  In  addition  to  statistics,  we  aimed  to  determine  the  similarities  and  differences  of health  systems and  the dimensions that  need  to be  developed in Turkey's  health  system  to bring some suggestions  to these  problems  experienced. For  this  purpose, life  expectancy at  birth and infant  mortality rates, which are  frequently  used in the  health economics  literature,  are  considered as  health  indicators,  while  health expenditures  and  share  of  GDP  are  considered  as  economic  performance  indicators.  Health  expenditures  per capita and  the share of  GDP  have increased  in  the last  40  years in  countries which  has same health  systems. However,  while health  expenditures  differ  in  countries with  the same health  systems,  it  is concluded  that  health indicators  and  health  systems are not  in  a closely  associated  with  their  success and  efficiency.
