Erythroderma is a severe, life-threatening condition that can arise in the context of various acute and chronic inflammatory diseases. In elderly patients presenting with erythroderma, particularly when mycosis fungoides is suspected, sequential biopsies and close clinical monitoring are essential. When histopathological findings and clinical presentation are discordant, other causes of erythroderma -especially adult-onset atopic dermatitis- should be considered. It is noteworthy that in patients over 65 years, atopic dermatitis may present predominantly with pruritus or erythroderma. Herein, we report a 77-year-old male with refractory atopic dermatitis and recurrent erythroderma, followed under suspicion of mycosis fungoides. A dramatic clinical response was observed within 5 days of upadacitinib initiation, and this response has been sustained for 15 months. Sequential clinical assessments and histopathological follow-up further substantiate the efficacy of therapy.
